MEMBERSHIP COMMITTEE
 
The Vancouver Division of Family Practice (VDoFP) currently has over 800 GP and resident members! Our membership continues to grow – nearly doubling in the past 18 months. We can proudly say that we now represent more than 80 percent of eligible practicing GPs in Vancouver and engaging our members continues to be a priority for the Membership Committee.

As an organization, the VDoFP and its members have matured substantially over the past two years. Through this growth, the Membership Committee has continued to develop and implement tools, resources, and events to enhance the practice life, and professional satisfaction of GPs. The VDoFP Clinical Dropbox Tool, launched April 21st is one example of the Membership Committee’s commitment to enhance practice efficiency and the patient point-of-care experience.

Doctor's Lounge

With the successful launch of the Doctor’s Lounge Pilot Series earlier this year, the Membership Committee is now planning more Doctor’s Lounge events to connect GPs and to help them improve their practice efficiency, give them an opportunity to experience informal mentorship through facilitated discussions.

Resident's Reception
The Committee also co-hosted this year’s Resident Reception on June 20th in partnership with the UBC Family Medicine Residence Program, and invited our resident and GP members to join the event. Residents received a "New to Practice Toolkit" and connected with VDoFP members and doctors in the community. We are also planning an insulin management CME event, based on the PSP model, to help our doctors become more comfortable with managing diabetic patients. Finally, we hosted our annual open house event at the Division office on July 15th at the division office where members and partners met staff and learned about our work.

CONTINUITY OF CARE LEFT

The Continuity of Care Committee (CCC) continues to pursue projects that support dialogue and cooperation between GPs and other healthcare providers. We are proud of our participation in the GP Care Conference design (as part of a broader Vancouver Coastal Home Health improvement project) to better link GPs with other home healthcare providers.

The CCC is also supporting projects for the upcoming flu vaccination program for improving GP-Cardiologist relationships.

We recently presented the results from our Pathways pre-feasibility study with the recommendation that the Vancouver Division pursue the deployment of Pathways while continuing to work with the Division Board and executives to determine the right parameters that will move the project forward. Project planning is well underway, and information and updates will be shared through FastFacts in the months to come.

Over the last few months, we also reviewed the BC Pharmacist Association's (BCPhA) proposals to expand pharmacist scope to ensure better patient care. We recommended to the Division Board that GPs work closely with BCPhA to ensure that the final proposals increase overall patient attachment.

The CCC is also supporting projects for the upcoming flu vaccination program and improving GP-Cardiologist relationships

As we continue to manage requests from partners in the community, the CCC will keep members informed of any opportunities that are available.

MENTAL HEALTH SUBCOMMITTEE


Announcement
St Paul’s Hospital staff have developed a fax that will be automatically sent to GPs when their patients are admitted to inform them that an admission has occurred. The form will also ask if and how a GP wishes to be involved in treatment.
 
 
 
VGH Psychiatry is setting up e-Mental Health space at the hospital. The space is comprised of computers with internet access that allows patients to complete coursework with the support of onsite staff. Patients will also be allowed to continue using the online tools once they're back in the community. Discussions of this tool are ongoing at the committee table and will continue along its development.

We're also happy to announce that the VCH/PHC Regional e-Mental Health Steering Committee has been awarded funding from the Shared Care Committee to develop an online e-Mental Health Hub for Depression and Anxiety. The Hub will be designed to improve access to information, diagnostic tools, and psychological treatments for patients with depression and/or anxiety, and to support clinicians in providing care to patients.
 
 
 
A patient experiencing mental health distress can face many stressors on their healthcare journey.

From seeing their family doctor to finally meeting an in-hospital psychiatrist, a patient can experience multiple interviews before getting the help they need.

The patient journey is being reviewed to map the patient journey from current state to future state in order to develop a better path for these patients.
 
 
 
A new Shared Care Clinical Model is being developed based on the Calgary approach and will be tested in Vancouver. A psychiatrist will start working in an existing clinic. Other possibilities include integrating into MedAccess and branching into another clinic.

INFORMATION TECHNOLOGY COMMITTEE

The IT Committee has an exciting year ahead with a strong focus on projects that will provide real value and return on investment. We are currently planning a number of initiatives that will assist and support your use of EMRs. They include:

  • Expanding the Train the Trainer program and planning for support for MOAs through EMR specific user groups.
  • Enhancing our communications to provide materials and support for EMR.
  • Holding workshops and seminars on data quality and standards.
  • Work related to the support The Pathways project (see CCC for more information).
We also continue to provide support through regular EMR User Groups: MedAccess, Intrahealth and Wolf. These meetings allow us to welcome both new and experienced users. The uptake of these sessions has been positive with increased attendance for several meetings. However, to hold these sessions we need a minimum of eight members to attend and on some occasions have been cancelled due to poor registration. Please send us your feedback so we can enhance these EMR sessions.

PRIMARY MATERNITY CARE NETWORK COMMITTEE
 

On May 8th, the Maternity Committee hosted its first in a series of CME events. The event received very positive feedback on the content, presentations and the overall format of the event and there are more events schedule for the fall. Future event dates:

  • September 18
  • October 15
  • November 18

RESIDENTIAL CARE COMMITTEEE

The Mentorship Pilot Project has just had its first applicants complete the program and the feedback that both mentors and mentees provided was overwhelmingly positive. We have introduced and mentored 5 new physicians to Residential Care work and assisted several physicians who are nearing retirement to find care for some of their residential patients. Once the project concludes, the Committee will review the results and determine what the next steps in mentorship work will be.
 
 
 
In the months to come, the Residential Care Committee has determined there is a need to focus more effort on the following two areas:
  1. Increasing the number of physicians that dedicate their practice, or a portion of their practice, to providing residential care.
  2. Improving the quality of the patient-provider experience.

We have partnered with the VCH Home Hospice program and PSP as part of this work, to develop a PSP-type module on Palliative Care in Residential Care Facilities. This is due to be presented in September of this year. Stay tuned to FastFacts for more information.

 

Get your copy of the Residential Care Baseline Research Project: Summary Report now from the VDoFP office.


The Residential Care Report, which discusses gaps in residential care services across Vancouver, was sent to residential care facilities in Vancouver and key stakeholders across the province. The VDoFP shared this report with the General Practice Services Committee (GPSC) Residential Care Provincial Working Group, who has been discussing residential care issues across the province over the past year. Through our VDoFP report, we were able to bring our perspective to the table and join the conversation of the group. The GPSC has already started looking at some solutions to issues highlighted in the report with the help and direction from the Committee.
